New Year, New…Documents???

I consider October the time of year where I review all my consent forms. Why? Most patients are expecting to redo forms in January anyways, this give me a chance to prepare the documents to be signed in January as well as give patients notice for any big changes coming their way. Here is a list of things I review in October:

  • Rates

  • Individual Therapy Informed Consent

  • Consent for Telehealth

  • Group Therapy Informed Consent

  • Relationship Therapy Informed Consent

  • Personal Disclosure Statement

If there are any changes, I notify all my patients verbally that a letter will be coming via the EHR about new year changes. I create a PDF letter titled “Upcoming <year> Changes,” and list all the changes. It is then uploaded into the EHR (we currently use Simple Practice but will be transitioning to Jane) for review. Then when January rolls around, consents are sent and signatures are obtained.
